L'edificio, iniziato nel 1284, è il più bel monumento della città; fu innalzato a ridosso dell'antica cinta muraria, di cui inglobò una torre, trasformata poi in campanile. Il Duomo si arricchì nel corso dei secoli di diversi capolavori, alcuni ancora conservati nel suo interno, altri trasferiti nella chiesa dei Ss. Giuseppe e Pantaleone, altri ancora andati perduti o rubati. Molti sono stati i lavori restauro e di trasformazione che hanno interessato il Duomo. Il 6 maggio 1976 la violenza del terremoto parve in pochi attimi metter fine a tutto: il Duomo resistette ma riportò danni gravissimi e solo la rapida solerzia con cui fu puntellato riuscì a salvarlo dalle successive scosse del 15 settembre. Il monumento era inizialmente concepito in modo diverso: nacque infatti sotto l'influsso dell'arte romanica , ma crebbe lentamente proprio in un periodo di transizione in cui si affacciò, fino a prenderne il sopravvento, l'arte gotica. Di concezione snella e lanciata, basato su equilibri arditi, il gotico diede al Duomo maestosità ed eleganza; fu forse completamente terminato verso l'anno 1420. Misura in totale 45 metri di lunghezza, 21 di larghezza e ben 20 metri in altezza. Veri gioielli del Duomo sono gli affreschi delle absidi, tutti del Trecento. I colori sono di un morbido pastello e su tutti prevale il verde. Le immagini, poi, nella loro apparente semplicità , emanano una sensazione di fiabesco, di naif. Nell'abside di sinistra sono raffigurati il Cristo Giudice, l'Annuncio ai pastori, la Natività, il Viaggio dei Magi, la Madonna in Trono, San Giacomo che resuscita l'impiccato e il Martirio di San Sebastiano. L'abside centrale è molto ricca e presenta il grande ciclo di affreschi che inizia sulla parete di destra con Storie del Vecchio Testamento (14 scene bibliche) e termina sulla parete opposta con le Storie del Nuovo Testamento (14 scene evangeliche); alzando lo sguardo nella volta è possibile ammirare dentro le vele ciò che rimane degli affreschi degli Evangelisti affiancati dai Dottori della Chiesa. Il ciclo di affreschi, dei quali ben leggibili sono solo quelli della fascia inferiore, coperta per secoli dagli stalli del coro ligneo di Marco Cozzi (1477), risulta strettamente dipendente da quello di VITALE DA BOLOGNA del Duomo di Udine di cui ripete i soggetti delle scene, il linguaggio stilistico, la tensione e la vivacità narrativa. Tra le due finestre dietro l'Altare s'innalza la Crocifissione, che esprime tutta la drammaticità della scena.
