L'edificio è a pianta basilicale, a croce latina lunga 102 metri e larga 40, a tre navate separate da due file di nove colonne ciascuna, tutte di granito ad eccezione di una, la prima a destra, che è di marmo cipollino: le colonne provengono da edifici romani, così come i capitelli con immagini di Cerere e di Proserpina tre foglie d'acanto e cornucopie. L'immensa navata centrale, tre volte più ampia delle due laterali, culmina nella grande abside maggiore coperta da una breve volta a ogiva, mentre le altre due absidi sono sovrastate da una semicupola, e una volta a botte e due a crociera ricoprono gli spazi ad esse antistanti. Il resto della chiesa ha soffitti lignei policromi che poggiano sulle pareti. Se la ricchezza di forme della basilica, la magnificenza dei suoi interni, la fastosità delle sue cappelle e degli arredi destano stupore e ammirazione, sono però i mosaici che rivestono tutto l'interno a suscitare meraviglia. La decorazione musiva si estende per 6.340 metri quadrati, ed è la più vasta d'Italia: l'hanno realizzata, probabilmente tra il 1180 e il 1190, squadre di mosaicisti bizantini affiancati da maestranze di altre scuole, gli uni e gli altri adeguandosi però ad un progetto di profonda unità poetica che rispetta una sua precisa logica sia nelle tappe figurative sia dal punto di vista teologico-dogmatico, con i tanti momenti dell'Antico e del Nuovo Testamento che culminano nell'abside centrale con la suggestiva visione del Cristo Pantokrator. Da una parete all'altra scorre la storia della salvezza: dai vari momenti della Creazione alle scene di Adamo ed Eva, di Caino e Abele, di Noè e dell'Arca; della torre di Babele e di Sodoma e Gomorra; e ancora, l'annuncio della venuta del Cristo, la sua nascita, i suoi miracoli, la sua morte e la sua resurrezione; gli apostoli, la loro missione nel mondo, sino alla solenne abside centrale dove con il Cristo ammiriamo le schiere dei cherubini e dei serafini, la Vergine, i santi...
