In presbiterio spicca, in tutta la sua imponenza, il grande altare tardo barocco realizzato da Pasquale Bocciardo nel 1764. La sua attuale lastra frontale è molto recente: risale al 1909. Prima di questa vi era una lastra molto più semplice, opera di Stefano Sormano (1603) che accompagnava le linee molto più essenziali e più sobrie dell'antico altare maggiore. Del 1604 sono le due aquile marmoree, poste ai lati dell'altare, che sorreggono le insegne della città di Savona. Esse costituiscono il solo elemento originale pervenutoci del primitivo altare e l'unico segno rimasto in loco del giuspatronato del Comune sulla cappella maggiore della cattedrale. Sulla struttura dell'altare insiste il monumentale ciborio (tabernacolo a forma di tempietto), significativo lavoro romano nell'ambito del primo barocco. Grazie all'interessamento di Giulietta Gavotti Ferrero e Violante Grassi Nano il progetto del ciborio era stato affidato al gesuita savonese Orazio Grassi (1583-1654), che lo aveva voluto con le stesse caratteristiche di forma e grandezza di quello che si trovava nella chiesa del Gesù a Roma, con alcune varianti e migliorie. Si richiedeva maggior pregio nella qualità dei marmi, nelle rifiniture e nelle dorature, e che venissero inserite nelle nicchie e sul prospetto rispettivamente statue di profeti e un rilievo raffigurante l'Ultima Cena, tutti in bronzo dorato, realizzati su modelli di Alessandro Algardi. Stipulato il contratto a Roma nel 1636 con l'impresario e mercante di marmi Santi Ghetti, il nuovo tabernacolo sarebbe stato allestito dal marmista Giovanni Piloti l'anno successivo, secondo gli accordi, e collocato entro il 1638 sull'altare maggiore in posizione centrale rispetto allo spazio del presbiterio, come segno tangibile del ruolo primario dell'Eucarestia nella vita di fede. Così infatti aveva voluto padre Orazio Grassi, indicando che questo tabernacolo doveva essere rifinito perfettamente su tutte le quattro facce, in modo che fosse visibile e apprezzabile da ogni punto di osservazione.
