Costruito in brevissimo tempo, usando materiali leggeri come il legno e lo stucco dipinti, il teatro nacque per volontà di Ranuccio I, duca di Parma e Piacenza dal 1593 al 1622, il quale intendeva festeggiare con grande sfarzo la sosta a Parma del Granduca di Toscana? Cosimo II de' Medici, allo scopo di instaurare un'alleanza politica attraverso un accordo matrimoniale tra le due famiglie ducali. Sfumato per motivi di salute il viaggio di Cosimo, l'inaugurazione del teatro ? già ultimato nel 1619 ? avvenne solo nel 1628, in occasione delle nozze tra Margherita de' Medici e il duca Odoardo, con uno spettacolo allegorico-mitologico dal titolo “Mercurio e Marte” (testo di Claudio Achillini e musiche di Claudio Monteverdi) arricchito da un torneo e culminante in una spettacolare naumachia. Data la complessità degli allestimenti scenici e i loro altissimi costi il teatro fu utilizzato soltanto nove volte, in occasione di matrimoni ducali o importanti visite di stato. Quasi completamente distrutto da un bombardamento aereo alleato nel maggio del 1944, il teatro è stato ricostruito tra il 1956 e il 1965 secondo il disegno originario.
