Fu costruita nel 1645 per volere di Madama Reale Cristina di Francia a ricordo del miracoloso salvataggio dal fiume di una bambina, avvenuto nel 1644 per opera dell'immagine della Vergine Annunciata apposta a un pilone votivo. Il dipinto divenne oggetto di diffusa devozione e la chiesa, con facciata coronata da alto timpano semicircolare, fu ingrandita nel 1779 e fornita di un battistero nel 1807. Ripetuti rifacimenti hanno quasi completamente trasformato l'interno, a una sola navata con due cappelle laterali, e della primitiva decorazione si conservano solo gli importanti stucchi attribuiti a Giovanni Andrea Casella e gli affreschi della cupola di Bartolomeo Guidobono. All'altar maggiore è l'immagine miracolosa dell'Annunciata del 1587, quasi completamente ridipinta nei restauri novecenteschi. Nel novembre 1994 la Chiesa e la piazza antistante furono invase dalle acque del fiume Po in piena, provocando seri danni alla struttura .
