Resta un mistero il motivo della presenza di due teste giganti di Medusa, che sostengono le colonne in uno degli estremi della cisterna. Secondo la mitologia greca, la Medusa era un mostro i cui capelli erano serpenti vivi e il cui sguardo trasformava in pietra chiunque la vedesse. Nella cisterna, una delle teste di Medusa è di profilo e l'altra a testa in giù. C'è chi afferma che si tratti semplicemente di sculture recuperate dalle rovine romane e collocate lì solo perché avevano la forma e le dimensioni adeguate a sostenere le colonne, senza altri significati. Altri storici, tuttavia, teorizzano che i cristiani che costruirono la cisterna misero le statue pagane a testa in giù di proposito, come modo per dichiarare la sconfitta del paganesimo.
