La cattedrale di San Giovanni Battista sorgeva anticamente, prima del terremoto del 1693, nell'antico centro della città di Ragusa. Dopo questo evento la Chiesa fu ricostruita nel cuore del nuovo centro abitato di Ragusa e la prima pietra fu posta il 15 aprile del 1694 e in solo 4 mesi la chiesa fu completata. Era una chiesa piccola che mal si adattava alle esigenze di una popolazione in crescita, infatti, nel 1718 iniziarono delle opere di ampliamento e restauro. Nel XIX secolo le navali laterali originarie vengono sostituite da piccole cappelle con all'interno degli altari e nello stesso periodo venne realizzata la pavimentazione di lastre di pietra pece con intarsi in calcare bianco.
