L'interno non conserva più nulla del suo primitivo aspetto, a causa dei grandi rifacimenti a cui la chiesa è andata soggetta nei secoli sedicesimo, diciassettesimo e, soprattutto, nel secolo diciottesimo. La navata centrale, con volta a botte lunettata che poggia su archi a tutto sesto sorretti da pilastri, incrociandosi con le navate laterali, forma una pianta a croce latina. Conserva preziosi reperti cosmateschi tra cui alcuni frammenti marmorei appartenenti ad un pergamo medievale, costruito nel 1222 in occasione della visita ad Alatri del pontefice Onorio III. Nel Duomo è conservata la statua del patrono della città, San Sisto I, martirizzato a Roma nel 125 sotto l'imperatore Adriano, il cui prezioso volto argenteo fu disegnato nel 1584 da Mons. Ignazio Danti; vi sono inoltre le reliquie del Santo pervenute in Alatri nel 1132 a dorso di una mula che ostinatamente, anziché proseguire il suo percorso verso Alife, dove era diretta, deviò verso Alatri fermandosi solo davanti alla Cattedrale. Da quell'anno esse sono custodite in questa Chiesa entro un'antichissima urna di piombo. Nella Basilica di San Paolo è anche custodito uno dei pochi miracoli eucaristici d'Italia: l'Ostia Incarnata. Una teca vitrea riposta a fianco della navata destra mostra la particola consacrata, divenuta straordinariamente carne umana in seguito ad un atto sacrilego compiuto in Alatri fra la fine del 1227 e l'inizio dell'anno successivo. La memoria di questo evento miracoloso è riportata in un mandato papale inviato da Gregorio IX al vescovo diocesano Giovanni V il 13 marzo 1228. L'interno complesso, costituito dalla Chiesa e dall'adiacente palazzo vescovile, racchiuso a sua volta da mura, è oggi circondato da una fascia a parco pubblico, dal quale si gode una vista superba.
