Il santuario si trova nella frazione Baragazza sull'appennino al confine con la toscana. Secondo la tradizione popolare il 16 luglio 1480, due pastorellli ebbero una visione della Madonna apparsa in corrispondenza del rio Davena "alla confluenza dei torrenti Tanone e Butrione da cui il nome della località boccadirio" e predisse loro una vita religiosa. Fu così che la popolazione di Baragazza decise di costruire nel 16° secolo una piccola chiesa con tabernacolo alla Madonna. La struttura attuale rispecchia abbastanza quella originale del 500. Solo nel 18° secolo furono costruiti il loggiato ed il chiostro su progetto di Aristotele Puccetti.
