La facciata, in stile tardo gotico-lombardo, è rivolta a ponente. Gli ultimi restauratori hanno individuato il colore originale della facciata, che non era solo di colore rosso mattone, perchè le formelle ritrovate sono policrome e decorate con fregi geometrici a fogli d'argento e d'oro. La ricostruzione della decorazione ha così portato i restauratori a ripristinare la presenza di fasce di esagoni rossi, chiuse da una greca e da decorazioni floreali, e di due grandi tau, la lettera simbolo dei Frati Antoniani, ai lati del rosone. La parte bassa è caratterizzata dall'ingresso a tre luci, ognuna di esse sormontata dalla caratteristica grande ghimberga decorata in cotto a motivi vegetali (soprattutto foglie di quercia e ghiande, nutrimento tipico dei maiali) e terminanti con pinnacoli. Il gusto tardo gotico ha portato all'esasperazione l'elemento delle ghimberghe, che in questa facciata occupano quasi tutta l'altezza dell'edificio e lo caratterizzano con forza. Alfredo D'Andrade durante il grande restauro del XIX-XX secolo aveva prelevato dalla facciata sessanta formelle in cotto, ordinandone la riproduzione al fine di preservarle, e per salvarle dall'imminente guerra le aveva nascoste nel chiostro. Poiché l'accesso al chiostro durante il conflitto finì a sua volta nascosto dalla vegetazione, delle formelle si era persa traccia: furono infine ritrovate nell'ultimo restauro del 2001. Questo tipo di decorazione rappresenta il punto di incontro tra creazione artistica ed esecuzione artigianale: infatti gli elementi decorativi venivano creati negli stampi e riprodotti in formelle, che consentivano di ripetere più volte i motivi ornamentali. Con questo sistema sono state così eseguite le cornici dei portali, le ghimberghe, le finestre, il rosone, i coronamenti del tetto, i pinnacoli, i fianchi della chiesa, l'abside, il campanile.
