L'Isola Tiberina, unica isola urbana del Tevere, lunga circa 300 metri e larga circa 90, è collegata alle sponde del Tevere da due ponti: verso Trastevere dal ponte Cestio, risalente al 46 a. C. e in direzione del Ghetto con il ponte Fabricio, edificato nel 62 a. C., chiamato anche ponte Quattro Capi, per le erme romane che ne ornano i parapetti. Secondo la leggenda, l'isola nasce nel 509 a.C. quando, spodestato Lucio Tarquinio Superbo, l'ultimo re di Roma, il popolo, in segno di odio verso il tiranno, gettò nel Tevere l'enorme deposito di grano del re, che risultò così abbondante da formare un'isoletta. In realtà, un compatto banco di tufo, simile a quello del vicino colle Capitolino, costituisce l'elemento geologico di base, sul quale naturalmente si sono sedimentate le sabbie condotte dalla corrente. Fin da epoca antichissima, la necessità delle popolazioni delle due sponde di guadare il fiume decretò l'importanza dell'isola, che assunse successivamente un carattere sacro. Un'altra leggenda racconta che, nel 291 a.C., quando la città di Roma era stata colpita da una terribile pestilenza che aveva mietuto moltissime vittime, i sacerdoti, dopo aver consultato i libri sibillini, avevano inviato una delegazione ad Epidauro, luogo di culto del dio della medicina Esculapio. Gli ambasciatori tornarono a Roma portando sulla nave un serpente, animale caro al dio. All'altezza dell'isola Tiberina, il serpente spiccò un salto e nel punto in cui si rifugiò, fu innalzato un tempio dedicato ad Esculapio; l'isola stessa fu sistemata architettonicamente come una nave con poppa e prua e nel mezzo un obelisco rappresentante l'albero maestro. Sui resti del tempio romano venne poi costruita la chiesa di San Bartolomeo all'Isola, che conserva un campanile romanico del XII secolo e una colonna scavata e utilizzata come vera da pozzo. L'antica vocazione medica dell'isola prosegue nel Medioevo e continua ancora oggi con l'ospedale “Fatebenefratelli”, nato nel 1584 e tutt'ora attivo, e l'ospedale Israelitico
