La Sagrestia di Sant'Agnese in Agone fu costruita tra il 1658 e il 1666 su disegno di Francesco Borromini che l'aveva progettata con tre assi di raccordo fra la Chiesa e il Collegio Innocenziano, inserita nella struttura di quest'ultimo palazzo. Dopo l'allontanamento dell'artista, fu terminata da Andrea Baratta. Vi si accede da un corridoio che da via dell'Anima immette nel braccio nord della Chiesa. Si tratta di un ambiente rettangolare con angoli convessi, noto come “sagrestia d'estate” che propone un impianto ecclesiale destinato alle funzioni private della famiglia e presenta la tipica quadripartizione dell antiche chiese paleocristiane: un vano d'ingresso, il nartece, l'aula e il presbiterio. Il nartece, con la nicchia per il lavamano in marmo realizzato dae Andrea Baratta è collegato alla navata da un grande arcone. L'aula è un vano ottagonale, con i quattro angoli convessi aperti da nicchie sovrastate da coretti e con la volta riccamente ornata con dipinti e stucchi dorati. Il presbiterio è un piccolo ambiente rettangolare (meno profondo di quello inizialmente previsto) con volta a botte, dedicato alla Vergine, con affreschi di Francesco Allegrini, rappresentanti le storie di Maria (1660): la Madonna sull'altare (l'odierna immagine dell'Immacolata è del secolo XVIII), la Natività di Maria, l'Annunciazione sulle pareti laterali, Angeli in adorazione nella lunetta della parete di fondo e l'Assunzione nella volta.
