Al suo interno ci sono 12 colonne di marmo e laterizio, mentre al centro si trova un'edicola che custodiva le reliquie di San Petronio, qui rinvenute nel 1141. La porticina del Sepolcro viene aperta una settimana l'anno, dopo la celebrazione della messa di mezzanotte di Pasqua, alla presenza dei Cavalieri del Santo Sepolcro. Anticamente era possibile strisciarci dentro per venerare i resti del santo; le prostitute di Bologna, la mattina di Pasqua, inoltre, in memoria di Maria Maddalena, vi si recavano per pronunciare, dinanzi al Santo Sepolcro, una preghiera il cui contenuto esse stesse non hanno mai voluto rivelare. Sempre secondo un'altra antica tradizione le donne incinte di Bologna solevano camminare trentatré volte (una per ogni anno di vita del Salvatore) attorno al Sepolcro, entrando ad ogni giro nel sepolcro per pregare; al termine del trentatreesimo giro, le donne si recavano poi nella vicina chiesa del Martyrium per pregare dinanzi all'affresco della Madonna Incinta.
