L'isola fu abitata fin dall'Età del ferro. Successivamente fu probabilmente una base militare etrusca ed anche sotto la dominazione romana fu una base di una discreta importanza nel Mar Tirreno citata, per esempio, da Giulio Cesare nel De bello civili e dal poeta Claudio Rutilio Namaziano. A margine dell'abitato di Giglio Porto, leggermente al di sotto del livello del mare, si trovano i resti della villa romana dei Domizi Enobarbi: si tratta di una vasta area che comprende una vasca a mare per la piscicultura, mura perimetrali e criptoportici, resti di opus sectile, mosaico, affreschi, terrazza stellata di pertinenza, strutture lungo mare con serie di arcate e una lunga terrazza pensile; l'intera area è denominata I Castellari. Nelle epoche successive fu governata da varie famiglie nobili dell'Italia centrale e dal 1264 dal governo pisano, che dovette poi cederla ai Medici. Nel Medioevo passò sotto il dominio della famiglia Aldobrandeschi, successivamente al comune di Perugia. Entrò nelle proprietà di Pisa dal 1264 al 1406 e successivamente passò a Firenze. Nel 1544 il pirata Khayr al-Din detto il Barbarossa saccheggiò l'isola, uccise chiunque si opponeva e deportò, come schiavi, oltre 700 gigliesi. In seguito a questa sanguinosa incursione, la famiglia dei Medici ripopolò l'isola con gente proveniente dalle terre senesi. Gli attacchi saraceni poi continuarono fino al 1799.
