Migliaia di teschi e ossa accumulate nel corso dei secoli durante epidemie e guerre. A metà strada tra fede e superstizione i devoti erano soliti "adottare" un teschio cui chiedere grazie e favori in cambio di preghiere che avrebbero dovuto alleviare la loro sofferenza nel purgatorio. Scelto il teschio, se ne prendevano cura, sistemandolo in una cassetta, ornandolo e facendogli visita ogni settimana. Ma se la grazia tardava ad arrivare ecco che il devoto lo trascurava e non esitava a cambiarlo con un altro. I teschi, ovvero "le anime pezzentelle" (anime dei poveri), furono continuamente accumulati nel corso dei secoli a partire dalla peste del 1656, ai quali si aggiunsero quelli delle epidemie successive, delle guerre e quelli rinvenuti durante gli scavi realizzati per l'edificazione della città. Attualmente si conta che nel sottosuolo del cimitero stesso,l'accumulo di resti continui per altri 4 metri di profondità. Nel 1969 l'arcivescovo di Napoli preoccupato del diffondersi di tale usanza, considerata profana, ne ordinò la chiusura. Dopo vari tentativi, soltanto nel 2010 è stato reso nuovamente fruibile al pubblico.
