Edificio del XVII secolo che, della preesistente chiesa del 1211, conserva soltanto il muro della parete sud. Appoggiata alla facciata, a sinistra della porta d'ingresso, una piccola cappella completamente affrescata. La Madonna con Bambino, S. Rocco e S. Sebastiano sulla parete di fondo, gli Evangelisti sulle vele della volta e i santi Nazario e Celso sull'arco sono opera, per la somiglianza e per l'epoca, degli stessi autori della chiesa del Castello a Semione: i Tarilli. All'interno della chiesa, la cappella della Madonna è ornata dagli stucchi di Gio Rezia da Porlezza e contiene nella nicchia una statua lignea dorata e policroma del secolo XVII. Sulla parete sud della sacrestia tre affreschi: a sinistra la Crocefissione e il sacramento della Confessione con cartiglio iscritto, a destra S. Vittore con la spada. La caratteristica cornice traforata che contorna la figura del santo, lo stesso tratto grafico, la linea di contorno dei personaggi della Crocefissione e infine l'analogia della figura di S. Vittore nelle chiese di S. Maria al Castello a Giornico, di S. Martino sui monti di Ditto e dell'oratorio di S. Bernardo a Monte Carasso, fanno attribuire questi affreschi ad uno stesso autore proveniente dalla bottega dei Seregnesi. La chiesa è stata restaurata negli anni 1995-98. Sono pure stati restaurati gli affreschi di Carlo Biucchi (scene della vita di S. Nazario, XVIII secolo), coperti un tempo da tele di E. Ferrazzini ora esposte sulla controfacciata.
