Il Duomo di Gaeta, dedicato a S. Maria Assunta in cielo fu costruito nei secoli X-XI probabilmente dove sorgeva una chiesa risalente forse al VII secolo (S. Maria del Parco). Tra la fine dell'VIII secolo e l'inizio di quello successivo, il vescovo di Formia si rifugiò nella fortezza di Gaeta e in quel momento fece nascondere in un pilastro della chiesa i resti di S. Erasmo, vescovo di Antiochia, morto a Formia il 2 giugno 303. Dopo oltre un secolo, il ritrovamento delle reliquie dette il via ai lavori per la costruzione di un'imponente cattedrale. Papa Pasquale II nel 1106 consacrò la nuova cattedrale a 5 o a 7 navate. Nel 1594, dopo la ricognizione canonica delle reliquie dei santi conservate in cattedrale si procede alla realizzazione della cripta destinata a contenere, sotto l'altare, le reliquie dei martiri: S. Erasmo, S. Marciano (primo vescovo di Siracusa), S. Casto (primo vescovo di Sessa Aurunca), S. Secondino (vescovo di Sinuessa), S. Probo (vescovo di Formia nel 303), S. Innocenzo (vescovo) e S. Eupuria (martire). Il monumentale campanile venne realizzato su disegno di Nicolò d'Angelo, marmorario romano della famiglia dei Vassalletto. Alto 57 metri, in stile arabo-normanno, ha il basamento composto da innumerevoli blocchi provenienti da monumenti romani e medievali (particolarmente dal mausoleo di Lucio Sempronio Atratino), gli altri livelli sono stati realizzati in laterizio con ricorsi di materiale lapideo, il torrino è decorato da innumerevoli scodelle arabo moresche. La scalea sottostante la torre campanaria da accesso alla chiesa: lungo la scala sono ben visibili due bassorilievi rappresentanti Giona che viene ingoiato e risputato dal pistrice, elementi facenti parte dell'antico pulpito della cattedrale. Sotto le sculture si trovano due sarcofagi romani strigilati (vasche della prima metà del III sec.). Il campanile è stato danneggiato nel corso dell'ultimo conflitto mondiale e restaurato nel 1960 e negli anni 1990-2000. La cattedrale è stata riaperta al culto nel 2014 a seguito di un restauro che ha restituito ai visitatori, almeno parzialmente, la struttura medievale sorretta da colonne e magnificamente decorata con arredi liturgici cosmateschi.
