Da una porticina si accede alla Cappella dell'Immacolata Concezione, nota come Cappella o Grotta d'oro, in funzione del cassettonato ligneo della volta dipinto ad oro. La cappella mostra un ciclo pittorico creando un unicum nel panorama artistico e architettonico del centro Italia. A pala d'altare vi è una pittura di Scipione Pulzone (1582), un'Assunzione della Vergine. Davanti questa immagine il Papa Pio IX, esule a Gaeta, ebbe l'idea del Dogma dell'Immacolata Concezione e da qui il 2 febbraio 1849 scrisse una lettera enciclica, la Ubi Primum, per consultare l'episcopato cattolico in merito alla definizione del dogma che sarà promulgato l'8 dicembre 1854. Lungo le pareti della cappella si svolge un racconto con storie della vita della Madonna e di Gesù. Il ciclo parte dalla parete di destra, a ridosso dell'altare, con la nascita della Madonna; prosegue nella finestra di fondo con la Profezia del Vecchio Testamento e ai lati l'Annunciazione dell'Angelo alla Madonna; ritornando accanto alla nascita di Maria, troviamo la Visitazione; passando alla parete di sinistra, a ridosso dell'altare troviamo la Nascità di Gesù e nella lunetta l'Adorazione dei Magi; sulla porticina trova posto il sogno di San Giuseppe; segue sulla stessa parete la fuga in Egitto, la strage degli innocenti, la circoncisione e la presentazione di Gesù al tempio; il ciclo prosegue sull'altra parete, a fianco della visitazione, con Gesù che insegna ai dottori della Chiesa, la scena lasciate che i bambini vengano a me e le nozze di Canaa. Nella penultima tavola troviamo un cartiglio con la data e la firma dell'autore, Giovan Filippo Criscuolo, 1531. Sull'altare, a sinistra dell'Assunta, sono San Rocco e San Pietro; a destra San Paolo e San Sebastiano. Rispetto alla totalità delle opere, il Natale del Signore e il Sogno di San Giuseppe sono da ascrivere ad un epoca successiva al Criscuolo. Il 25 marzo 2009 l'Arcivescovo di Gaeta, Mons. Fabio Bernardo D'Onorio, ha elevato la chiesa dell'Annunziata a Santuario Diocesano gemellato con Lourdes.
