La chiesa di San Paolo fu eretta prima dell'anno mille sui basamenti di un'area sacra "pelasgica", di cui restano evidenti segni lungo il lato settentrionale della costruzione. Si presenta attualmente con le forme assunte nel corso del XVIII secolo, in seguito a consistenti ristrutturazioni che culminarono con la costruzione di una nuova imponente facciata, in pietra e laterizio, affidata al progetto dell'architetto Jacopo Subleyras, eretta insieme al vicino campanile e terminata nel 1808. La facciata mostra di ispirarsi al modello delle maggiori basiliche romane, per la presenza di un unico ordine di paraste a binati. Dello stesso architetto sono la ricostruzione della collegiata di San Michele Arcangelo a Boville Ernica, con facciata in mattoni e due ordini architettonici in pietra. Lo stesso architetto ridisegnò il fornice della Porta San Francesco, collocata sul lato occidentale della cinta muraria di Alatri, che costituiva originariamente l'accesso del decumano massimo di epoca romana. L'attico e il timpano della chiesa furono invece aggiunti nel 1883, su progetto di Giulio Pitocchi. Dal pronao, pavimentato con ciottoli di fiume, attraverso tre porte, corrispondenti alle navate, ci si immette nella basilica.
