E' composto da 3 sacelli absidali, ai quali si accede dalla navata centrale, presso il pilastro di destra, scendendo 12 antichissimi scalini assai logori, che fanno pensare alle migliaia di pellegrini che li calpestarono fin dal lontano Medioevo. Le ipotesi circa le origini del primitivo santuario sono numerose, si intrecciano e restano tali, mentre le fonti mancano o sono insicure. Ma gli studiosi concordano nell'individuare qui la prima Sacra ed il momento storico originario del suo culto a Michele. La cappella più vasta e con parete di fondo in viva roccia è un ampliamento delle altre due, ed è ora dedicata a San Giovanni Vincenzo. Siamo nel luogo più sacro della Sacra.
