Il Santuario romanico-gotico che accoglie oggi il visitatore alla sommità del monte, venne realizzato e modificato nel corso di più secoli. Vi appaiono tre generi di architettura: uno romanico nella parte absidale, orientata verso il punto esatto in cui sorge il sole il giorno della festività di San Michele (29 settembre), nella prima arcata e relative finestre e colonne; uno romanico di transizione nelle due successive arcate con pilastri a fascio e archi acuti, e uno gotico di scuola piacentina nella decorazione del finestrone dell'abside centrale e nelle due finestre delle navate minori. L'inizio dei lavori di costruzione della chiesa è di difficile datazione, ma si suppone che l'avvio sia stato commissionato dall'abate Stefano (1148-1170). Originariamente la chiesa doveva essere sormontata da volte a crociera analoghe alle attuali. Tali volte crollarono e, nel Seicento, furono sostituite, nella navata centrale, da una pesante volta a botte, che esercitava una notevole spinta sui muri laterali, minacciandone la stabilità e creando pericolo di ribaltamento. Per far fronte a questa minaccia, durante i restauri di fine Ottocento, fu demolita la volta a botte e sostituita con una triplice volta a crociera completata nel 1937. All'interno della chiesa imponenti colonne, numerose colonnette, lesene, spigoli?il tutto coronato da suggestivi e simbolici capitelli: se ne contano 139. Di particolare interesse il primo pilastro a sinistra della navata centrale, sotto il quale affiora per 15 centimetri la cima del monte Pirchiriano, "culmine vertiginosamente santo", come lo definì il poeta-rosminiano Clemente Rebora.
