L'altare attuale è stato voluto da Carlo III (1759-1788), progettato da Francesco Sabatini (1722-1795). E 'iniziato nel 1768 e fu completato nel 1775 nei laboratori del Palazzo Reale di Madrid. La pala d'altare che occupa l'altare maggiore è un insieme di marmi e bronzi che ricreano una struttura neoclassica. La cancellata che racchiude l'altare è un'opera barocca di Antonio Elorza (1736) e le dorature di Miguel e José Borbúa di Segovia ed è decorata con gigli, stemmi e medaglioni sulle creste.
