Durante l'occupazione napoleonica l'edificio ospitò i soldati francesi e fu gravemente danneggiato, nel 1906 qui si celebrò il matrimonio tra Alfonso XIII e Vittoria Eugenia di Battenberg, e sempre qui Juan Carlos I è stato dichiarato re nel 1975. Oggi la chiesa ospita le nozze, le proclamazioni reali ed i giuramenti dei principi delle Asturie.Al suo interno si trovano alcune opere di grande valore artistico, come sculture di Benlliure e Juan Pascual de Mena. All'interno di un'ala, di recente costruzione, del vicino Museo del Prado è possibile inoltre ammirare l'antico chiostro della chiesa, i cui 2.820 blocchi sono stati smontati, restaurati singolarmente e rimontati nella nuova sede.
