L'origine della chiesa è legata alla Compagnia dei Battuti, presente a Bologna dal 1260 e dedita all'accoglienza dei pellegrini e alla cura dei malati. Distrutto da un terremoto nel 1686, l'edificio fu interamente ricostruito. Nel 1787 venne completata la cupola, su disegno di A. Galli Bibiena, mentre la facciata fu realizzata nel 1905. All'interno: la Madonna della Vita, dipinto del XIV-XV secolo, e il celebre Compianto di N.
