L'interno ha un'unica grande navata ellittica - 50 metri per 26- sormontata da un'alta cupola elissoidica di notevole arditezza. Due grandi aperture ad arco si fronteggiano; in corrispondenza, una della porta di ingresso e l'altra del presbiterio. Sull'arco d'ingresso si trova un bell'organo, con bassorilievi di soggetto musicale. Su ciascuno dei due lati si aprono tre arcate che immettono nelle sei cappelle degli altari laterali (ognuna a pianta rettangolare e con volta a botte). Si alternano alle arcate, a stucco con venature che ricordano il marmo, gruppi di due colonne anch'esse stuccate a marmo verde di Susa, coperte da capitelli bianchi, ionici, ornati di ghirlande, che reggono l'elegante ed imponente trabeazione corrente tutt'intorno alla chiesa. Alle colonne corrispondono delle nervature che scandiscono la volta. Divide la navata dal coro una semplice balaustra marmorea. Il presbiterio e il coro rivelano un'impronta già neoclassica. Sormonta il presbiterio una cupola a base circolare; ai fianchi corrono due tribune sostenute da colonne doriche. Il coro, semicircolare, è ornato da sei colonne incastrate con capitelli ionici e ghirlande, e coperto da una volta a semicatino. La decorazione delle volte di tutta la chiesa è opera del Sereno, che nel 1871 dipinge a colori accesi le storie della Vergine, con risultati non all'altezza della splendida architettura. Maestoso si erge l'altare marmoreo, nelle pietre verdi e arancio copiate dal decoro della chiesa Ha ornamenti di ghirlande ed eleganti putti, in marmo bianco ai lati, in legno dipinto al centro, sopra il tabernacolo. E' sormontato da una semplice e leggera croce e da sei imponenti candelabri. Nel coro vi è una copia - non paragonabile all'originale della tela più importante della chiesa, ora collocata in posizione nascosta: la Natività della Vergine, che fu commissionata per rappresentare l'intitolazione della chiesa e che per molti anni ha ornato il coro sopra l' altare maggiore. La tela rappresenta, con vivacità espressiva e attenta costruzione della scena, Sant'Anna con la Madonna appena nata e alcune figure femminili ritratte in gesti ed espressioni di vita quotidiana. Sullo sfondo compare la figura di San Gioacchino, l'unico ritratto in un atteggiamento stereotipato di devozione. L'opera potrebbe essere attribuibile ad Antonio Milocco, pittore di scuola beaumontiana.
