E' la gemella di Santa Maria dei Miracoli, pur presentando alcune differenze soprattutto planimetriche; entrambe conferiscono a piazza del Popolo una caratterizzazione in pieno stile barocco. Mentre Santa Maria dei Miracoli presenta una pianta circolare, Santa Maria in Montesanto mostra una forma ellittica. Progettate per volontà di Papa Alessandro VII, sono entrambe dedicate alla Madonna e sono state erette proprio all'inizio della via Lata (odierna via del Corso), perché fosse la Chiesa, con la protezione materna della Vergine Maria, ad accogliere i pellegrini che dalla Porta Flaminia entravano nella “città santa”. La costruzione si deve alla munificenza del Cardinale Gerolamo Gastaldi.
