Su questo altare barocco è situata un'icona della Vergine molto venerata dai romani. L'altare fu fatto costruire in occasione del giubileo del 1600 per incorniciare e proteggere la venerata immagine Si hanno sue notizie sin dall'ottavo secolo ma, perduto l'originale, fu sostituita da una copia nella metà del X secolo. L'originale, proveniente da Gerusalemme, era custodito nel santuario di Chalcoprateia e arrivato dalla capitale bizantina a Roma nel V secolo. Maria guarda l'osservatore leggermente voltata verso destra, la sua mano destra è alzata mentre la sinistra è appoggiata al petto ad indicare che intercede per tutti coloro che si rivolgono a Lei. Si attribuisce a questa immagine dell'Aracoeli, come abbiamo detto, la cessazione della peste a Roma nel 1348 e il 30 maggio 1948 le fu consacrata la città di Roma.
