La chiesa Collegiata sorge sul posto dove si trovava l'Oratorio della Trinità (sec. XIII), come ci riferiscono l'Allevi in "A zonzo per Offida" e l'Arduini in "Memorie Historiche della città di Offida". L'Oratorio, con le sue statue e pitture, fu demolito sul finire del sec. XVIII e al suo posto sorse la nuova Collegiata, consacrata a Maria Assunta in cielo. I lavori di costruzione iniziarono nel 1785, utilizzando parte del materiale ricavato dalla demolizione del monastero di S. Maria della Rocca, sotto la direzione di Pietro Maggi, ticinese, e su disegno di Lazzaro Giosafatti (1694-1781).
