La cappella fu eretta verso la fine del sec. XVII in sontuoso stile barocco su progetto dell'architetto scultore Filippo Parodi. Del Parodi sono anche le statue sulla balaustra (da sinistra: S. Francesco d'Assisi, la Fede, l'Umiltà, la Penitenza, la Carità, S. Bonaventura) e sopra la nicchia centrale (S. Antonio in gloria). Nelle tre nicchie sono esposti decine di reliquiari, calici, ex-voto, autografi di santi, ecc. Nella nicchia di sinistra, è conservato un moderno reliquiario contenente una reliquia di san Giovanni Paolo II. Il prezioso reliquiario centrale di R. Cremesini (1982) contiene un osso del piede, un frammento di cute e capelli del Santo. Nella nicchia centrale, in un mirabile reliquiario dell'orefice Giuliano da Firenze (1436), è conservata la Lingua incorrotta di sant'Antonio; più sopra, il reliquiario del Mento del Santo, opera d'artista padovano (1350); in alto il reliquiario della croce di cristallo, capolavoro di G. Fabbro (1437); subito sotto la Lingua, il moderno significativo reliquiario di C. Balljana (1981), che racchiude l'apparato vocale del Santo, ritrovato nell'ultima ricognizione. La cappella custodisce anche la tonaca di sant'Antonio e la cassa in cui era stato deposto il corpo.
