L'immenso spazio interno, lungo 186,36 metri (la scritta all'ingresso riporta 837 P.R. che sta per palmi romani), è articolato in tre navate per mezzo di robusti pilastri sui quali si aprono grandi archi a tutto sesto, alti 23 metri e larghi 13. La superficie calpestabile è di 15160 metri quadrati. La navata centrale è lunga 90 metri (dalla controfacciata ai primi pilastri della cupola), larga 26 metri e alta circa 45 metri e da sola copre circa 2500 metri quadrati di superficie. È coperta da un'ampia volta a botte e culmina, dietro al colossale Baldacchino di San Pietro, nella monumentale Cattedra. Planimetria Particolarmente ricercato è il disegno del pavimento marmoreo, in cui sono presenti elementi provenienti dalla precedente basilica, come il disco in porfido rosso egiziano sul quale si inginocchiò Carlo Magno il giorno della sua incoronazione (la cosiddetta Rota Porphyretica). Il pavimento marmoreo sostituisce quello precedenti in mattoni (quest'ultimo inizialmente presente solo nel corpo aggiunto da Maderno) e fu realizzato da Gian Lorenzo Bernini per il giubileo del 1650, assieme alle decorazioni della navata. Diecimila metri quadrati di mosaici rivestono poi le superfici interne e si devono all'opera di numerosi artisti che operarono soprattutto tra il Seicento e il Settecento, come Pietro da Cortona, Giovanni De Vecchi, Cavalier d'Arpino e Francesco Trevisani.
