Il ciborio, opera di Arnolfo di Cambio, è uno dei pochi elementi "sopravvissuti" all'incendio che, nel 1823, ridusse in cenere l'intera basilica di san Paolo. Commissionato dall'abate Bartolomeo (che ricoprì tale carica tra il 1282 e il 1297), venne portato a termine dall'architetto e scultore nel 1285 e fu il primo lavoro eseguito dall'artista toscano a Roma. Lo scultore vi lavorò a quattro mani. Il baldacchino reca infatti la firma dell'esecutore “cum Petro suo socio”. Ne è ancora incerta l'identità; forse si trattava di Pietro Cavallini, che si stava dedicando, in quello stesso periodo, all'opera di restauro degli affreschi del Vecchio e Nuovo Testamento della navata centrale; forse (ed è l'opinione prevalente) era lo scultore romano Pietro di Oderisio, che a Roma aveva già eseguito il monumento al pontefice Clemente IV (spostato poi nella chiesa di San Francesco a Viterbo). Il baldacchino di san Paolo nasce come rielaborazione del modello tipico della tradizione romana: più slanciato in verticale e con linee e forme derivate dal gotico francese, l'insieme venne arricchito di statue e bassorilievi, con intarsi cosmateschi policromi. Le quattro colonne che sorreggono il baldacchino sono in porfido e terminano con capitelli in marmo dorato. Le arcate sono trilobate a sesto acuto e contribuiscono alla spinta verticale, assieme ai quattro frontoni triangolari affiancate da guglie. Questo moto verso l'alto è accentuato anche dalla struttura a forma di tempietto, che culmina in una cuspide piramidale. Le nicchie in cima alle colonne ospitano statue di santi. Le statue di Pietro e Paolo poste «alle estremità del lato occidentale accolgono il devoto pellegrino che si avvicina alla mensa di Cristo. Caratterizzate da una forte impronta classicheggiante nella postura, le sculture sono entrambe avvolte in pesanti vesti, che definite da pieghe dritte dal ritmo sobrio e realistico, aderiscono alla struttura dei corpi, assecondandone i gesti. La figura di Pietro, nobile e solenne, leggermente ruotata, volge lo sguardo fiero verso i fedeli al centro della navata. Nell'effige di Paolo, meno saldamente costruita, più atona e immota, prevale l'andamento lineare del panneggio che rivela una figura più esile e spigolosa» (Sante Guido). Sono presenti anche le statue di Timoteo, discepolo di Paolo, e di san Benedetto, che tuttavia non è rappresentato secondo la classica iconografia del santo con il volto barbato, ma il cui viso rappresenterebbe invece lo stesso abate committente del baldacchino. “Il bellissimo volto – commenta Sante Guido – si rivela un efficace esempio dell'abilità ritrattistica arnolfiana, in grado di cogliere con acutezza il carattere placido e mite di Bartolomeo”. Il lato occidentale ospita la scena che raffigura l'offerta del baldacchino a san Paolo. L'abate Bartolomeo, affiancato da un monaco, appare inginocchiato mentre presenta la miniatura della preziosa struttura architettonica al Santo. Nel prospetto orientale sono invece rappresentati David e Salomone. Nei pennacchi posti in corrispondenza del lato settentrionale, viene narrato l'episodio del peccato originale. Nel quarto lato è raffigurata l'offerta di Caino e Abele, che si presentano a Dio con i doni, frutto del loro lavoro.
