La basilica è caratterizzata da una pianta variegata e unica nel suo genere. Il grande corpus ellittico anticipa la navata principale a pianta esagonale, con due coppie di cappelle ogivali che affiancano la cappella centrale, con funzione di abside, dove è collocato l'altar maggiore, disegnato da Filippo Juvarra. L'antica cripta, originariamente posta sotto il presbiterio, corrisponde alla attuale "Cappella delle Grazie", ed è visibile dal loggiato realizzato anch'esso nell'ultimo ampliamento del XX secolo. Appena entrati, si è accolti dall'ampia aula detta "di Sant'Andrea", riccamente decorata e in cui sono presenti tre altari laterali (a sinistra la Cappella di Sant'Anna, con la pala d'altare di Rapous, e la cappella mariana di Sant'Andrea, a destra la Cappella del Cafasso), più i passaggi alle altre aree interne. Il corpus centrale invece, è detto "esagono guariniano", essendo stato rimaneggiato dal Guarini sul finire del XVII secolo: Nella volta il Trionfo di Maria Assunta di Mattia Bortoloni entro quadrature di Felice Biella con otto medaglioni alla base con apostoli e virtù teologali.
