Il monumento sacro più importante di Terracina è sicuramente il Duomo, dedicato a San Cesareo e Concattedrale della diocesi di Latina-Terracina-Sezze-Priverno. Le origini del Duomo risalgono ad un periodo che va dal V al VI secolo. la realizzazione dell'edificio sacro è avvenuta tramite il riutilizzo di materiale proveniente da un antico tempio romano del foro cittadino. Nel corso dei secoli il monumento subì notevoli interventi di restauro, tra cui il più importante è quello dell'XI secolo, al termine del quale l'edificio venne consacrato a San Cesareo. Un altro intervento da ricordare è quello del XVIII secolo, che ridusse l'interno del Duomo da cinque a tre navate, costruendo cappelle laterali e sostituendo la copertura a capriate romaniche con l'odierna copertura a botte. Il duomo di Terracina riveste una notevole importanza nella storia della Chiesa, in quanto è stato sede del conclave del 1088 che portò all'elezione di papa Urbano II. Di interesse architettonico e artistico è il porticato che precede la chiesa, elevato al di sopra di una gradinata composta da ben 25 gradini. Un campanile romanico è posto ai lati del porticato, ed è sollevato da terra, sorretto da quattro pilastri. All'interno del Duomo di San Cesareo è di interesse la pavimentazione della navata centrale, realizzata in stile cosmatesco e il pulpito medievale, sorretto da cinque colonne. Da vedere sono gli affreschi settecenteschi del presbiterio e del coro, riportanti alcuni degli episodi salienti che hanno portato all'elezione di papa Urbano II.
