Il primo documento che riguarda il fabbricato della chiesa è del 1667, in occasione della visita pastorale del Vescovo di Alba, Monsignor Vittorio Della Chiesa. In quell'anno l'edificio risulta edificato per due terzi e con la parte del coro (rivolta ad est) ultimata; all'esterno dell'abside, a circa 8 metri dal suolo, una grossa pietra reca la data incisa: 1662, anno di inizio dei lavori di ricostruzione. La croce incisa a lato indica che la chiesa era consacrata quindi vi si potevano svolgere le funzioni, benchè non ancora ultimata. Fin dall'anno 1600 la parrocchia aveva assunto il titolo di Nativita' della Beata Vergine Maria; da quello più generico di Santa Maria, si era scelta una titolazione che facesse riferimento ad un momento particolare della vicenda terrena di Maria, a cui poter rivolgere preghiere, magari per avere figli sani e numerosi, unica vera ricchezza per un popolo contadino. La chiesa è in stile barocco piemontese, decisamente più sobrio rispetto ai noti esempi di architetture romane, torinesi o veneziane coeve; costruita ad aula su un piccolo colle, domina il borgo. L'altare maggiore è uno degli elementi più antichi della Chiesa, realizzato in marmi policromi nel 1872, in sostituzione di quello costruito da Antonio Pedruzzi e Domenico Cometa nel 1670: questa data è incisa nel piccolo armadio in muratura sul retro dell'altare. Ciò dimostra che il più antico scheletro dell'opera è stato inglobato in quello ottocentesco. Sopra l'altare è stato sistemato un crocifisso tardo-medioevale, proveniente dall'antico oratorio dei Battuti. Al di sopra ancora era sospeso un scenografico baldacchino a pendoni in legno dipinto (oggi ricoverato nella tribuna della controfacciata) messo in opera nel 1781. Alle spalle dell'altare è ancora ben conservato il coro ligneo con leggio centrale, già presente nel 1667. Al fondo dell'abside è collocata la pala d'altare con la Natività della Vergine, ora in corso di restauro presso il laboratorio di Claudia Maritano, Carcare. Ai lati della pala, due finte nicchie con effetto trompe l'oeil nei toni del grigio raffigurano i Santi Pietro e Paolo; opera del Borgna. Sulle pareti laterali del presbiterio sono collocati due dipinti: la Visitazione dei pastori e l'Adorazione dei Magi. La volta dell'aula è affrescata dal pittore savonese Giovanni Borgna nel 1888, con la Gloria (Assunzione) di Maria al Cielo. il gruppo ligneo della Madonna del Rosario è opera del savonese Filippo Martinengo, scultore di due casse processionali per il Venerdì Santo di Savona. L'opera gli venne commissionata nel 1786. Sono in corso importanti restauri.
