Questa è la cappella dedicata alla protettrice principale della città di Matera: Maria Ss. della Bruna. L'immagine della Vergine, posta al centro dell'alzata marmorea, è un affresco del XIII secolo attribuito a Giovanni da Taranto. In origine era collocato in controfacciata, a sinistra dell'altare, nel luogo ora occupato dal busto dell'Arcivescovo Antonio Del Ryos. Nel 1576, per volere dell'Arciprete Giovan Pietro Sanità, l'affresco fu tagliato con l'intera catena di pietre e murato nella posizione attuale, ma poco più in basso. Nel 1706 l'Arcivescovo Brancaccio commissionò al napoletano Francesco Cimafonte un nuovo altare di cui rimane solo l'alzata. Questa si compone di una cimasa con la tela della Visitazione di Maria a Sant'Elisabetta, di quattro lesene e di due nicchie entro cui sono collocate le statue marmoree di Re Davide e del Profeta Isaia che reggono cartigli con citazioni del Vecchio Testamento. L'altare propriamente detto, commissionato nel 1731 dall'Arcivescovo Alfonso Mariconda al marmorario napoletano Cristoforo Battimelli, quale altare maggiore della Cattedrale, venne trasferito dal presbiterio nella posizione attuale nel 1786 per volontà dell'Arcivescovo Francesco Zunica. Si tratta di un'opera particolarmente pregiata, costituita da splendida tarsia di lapislazzuli, madreperle, marmi pregiati e cristalli di rocca che formano una ricca varietà di fiori.
